Most likely an error of a plugin i would guess, that is if you havent touched any code yourself. Try to disable plugins one by one to see if its one of them. Could think of screenshots, or facebook who might could cause this. However by default they dont.

Ok guys i want to test out a simple method to stop a few, yes just a few, as you cant block every proxy. Especially with a htaccess or php method which checks the headers, its almost impossible to block as most fake/remove the needed headers. This method will work out for proxies not running...

yea analytics plugin has nothing to do with it The file goes to topsite root where index.php is. Or the meta snippet into the head area of wrapper.html

check if your on right url if your list is site.com but you access www.site.com it can lead to session issues
