Broken banner image

Any ideas why i get a broken banner image?

I got one 125x125 showing fine but my 468x60 isn't?
From a user point of view. You know when you click on LINKCODES as a user?

Both banners are uploaded into http://www.mysite.com/images/banners/
Under my settings i got http://www.mysite.com/images/banners/ setup as path
GIF selected as extention (yes both are gif images) In fact if i type in the full urls of the banners they work fine
and 2 images selected (well 4 as i got 4 in there for real but won't use them for this)

UPDATE:
Ok it's working fine if the user as ONE site to choose from, to grab the banner for.
On my test i had to 2 sites added under the same user. It's the second one that shows a broken banner. How come? In other words,

which is banner 125x125 works fine

banner 468x60 is broken

Both are working fine under the first site to choose from.

UPDATE

Sometimes the 468x60 banner shows up and sometimes it doesn't?
It has nothing to do with the number of links added under an account like i said in the above post.

I created 4 accounts and added some links in each of them and some are working and some are not.
I mean when you click on LINKCODE under the respective accounts, to grab the linkcode/banner.

Sometimes you have the option to choose the 468x60 to promote your link and sometimes it's a broken image?
The first option my 125x125 always shows up

Any ideas why it's doing this?

This sounds like an issue that is isolated to your site, you would need to provide a URL where we can see this happening.
