Error on create new category

When I go to go the create a new category I get the following message at the top of the page.

Looks like maybe something is wrong with your skin. If you look at the path "skins/index.htm/info.php" its clearly wrong. Index.htm should not be there in the path at all.
What skin you use?
What does stand in the info.php?
did it happen before 0.7 beta as well?
